O positive blood—commonly written O+ or O Rh-positive—is a blood type defined by two antigen systems: the ABO system, in which it belongs to group O, and the Rh system, in which it carries the positive factor. Group O means that red blood cells lack both A and B antigens (antigens are protein or glycoprotein markers on cell surfaces that the immune system can recognize). The term "positive" indicates the presence of the Rh D antigen.

O+ is among the most common blood types globally, particularly in Africa, Asia, and the Americas. Prevalence varies by country and population, so global figures should be taken as approximate rather than exact.

Definition

The ABO system is based on antigens present on red blood cells. A person with group O lacks both A and B antigens, but their blood typically contains antibodies against both A and B. The Rh system refers to the D antigen. Those carrying D are Rh-positive; those without it are Rh-negative. Combining these two systems produces designations such as A+, B−, AB+, and O+.

Blood transfusion

Red blood cells from O+ donors can be given to anyone who is Rh-positive, regardless of their ABO group—but not to Rh-negative recipients, since the D antigen can trigger an immune response. For this reason, O+ is often used in emergency situations when the patient's blood type is unknown, particularly for adult men and women beyond childbearing age. When Rh-negative blood is needed or known to be required, O− is selected instead. A person with O+ blood can receive only red cells of group O (O+ or O−).

NoteThe term "universal donor" applied to O refers to red blood cells only. Plasma follows different rules: AB is considered the universal plasma donor.

Geographic distribution

Group O is generally more common than other types across much of the world, while Rh-positive is nearly universal in most populations. Rh-negative prevalence is highest among people of European descent; in Asia and Africa it is considerably lower. Accordingly, O+ is important for blood bank planning: both demand and supply tend to center on this single type.
