Substance dependence is a medical and behavioral condition affecting the brain and conduct, in which a person's use of substances such as alcohol, tobacco, khat, cannabis, opioids, or sedative medications becomes difficult to stop, despite resulting harm.
Core features include intense craving, loss of control over the amount consumed, continued use despite negative consequences, and prioritization of use over other activities such as work or family. Tolerance is commonly observed—the need for increasing doses to achieve the same effect—as are withdrawal symptoms when use stops.
Mechanism of dependence
Dependence-producing substances affect brain systems involved in reward, particularly the dopamine pathway. Repeated use trains the brain to associate contextual cues—places, people, stress, or mood—with craving. Simultaneously, the body adapts to the substance's presence, making stopping unpredictable and withdrawal symptoms possible. Both learning and biological change are involved.
Risk factors
Risk varies among individuals. Factors include family history, age of first use—earlier onset carries greater risk—mental health conditions such as depression and anxiety, and social circumstances: unemployment, conflict, homelessness, and ease of access to the substance. No single factor determines outcome; typically multiple factors combine.
Consequences
Effects span physical health (liver, heart, lungs, infectious disease from shared injection equipment), mental health, and family and financial relationships. Some substances carry direct overdose risk, particularly opioids when combined with sedative medications.
Treatment and management
Treatment typically involves two components: management of withdrawal symptoms, sometimes requiring medication, and longer-term therapy including counseling, cognitive-behavioral therapy, peer support groups, and social reintegration—employment, housing, and relationships. In certain conditions, such as opioid dependence, medication-assisted treatment is used.
